a company within the Florida Construction Licensing Board's realm, you will need
a credit history and background check to send in with your license. MacData Advantage

report will be returned to you within 24 hours Monday through Friday. PLEASE
NOTE: Once MacData Advantage has completed the report, we will either fax
or email a copy of your report as you requested us on the release sheet and mail
a copy to be included with your application. Do not open the mailed copy,
as the state wants a sealed copy with your application. In
compliance with the Fair Credit Reporting Act, your signature is required in order
for us to complete your order. Please print the Notification/Release of Information,
fill it out, sign it, and fax it to MacData Advantage. Make sure you advise
us whether to fax or email your copy to you.
